See the kernel doc of the particular kernel version:
/usr/src/linux/Documentation/devices.txt

FYI, this will all change if you run devfs. I guess Linus got tired of
all the maintenance and other requirements of having a special device
with fixed name for every piece of hardware. devfs is intended to be
something of a dynamic scheme (it'll cause some surprises if you use it
and don't expect it).
