Are you running the latest UW-IMAP patch from RedHat?

Additional note from maillog, when connecting by working means it logs
the user name correctly, when connecting via IMP it names the user as
"???" (and also does not find the host), so it is failing to send the
user name. I need to figure out how to tell it to correctly get the name
of the user it would seem. I am entering the user in the username
prompt, but apparently it is ignoring it or not passing it on, it says
something similar to:
  imapd[3581]: Command stream end of file, while reading line user=???
host=UNKNOWN

The connections that do work say more like:
  imapd[3567]: Authenticated user=stimits host=localhost [127.0.0.1]

What would cause it to fail to not pass along the user name I give it,
as well as failing to know what host?
