It really depends on the driver and the exact failure.  I've seen the
behavior you mention, but not in the last few years.  I've also seen drives
fall out of the array correctly and the system go on it's merry way in
degraded mode.

Yep, that's also what we do.  Some people think they don't have time to set
up array monitoring...  If you don't have time to do it right, you need to
outsource some of your IT.  Hint hint.  :-)
