[lug] Sendmail, access, and "duplicate key" error.

I'm trying to 

makemap hash -o access.db < access

and I keep getting "duplicate key" errors. 

It does it for every entry:

localhost.localdomain	RELAY
localhost		RELAY
domain.cxm		RELAY

Can anyone tell me where to look for this.
