2.  A copy of the latest completed official performance appraisal with
rating.

NOTE FAILURE TO SUBMIT THE REQUESTED FORMS MAY RESULT IN A LOWER RATING IN
THE EVALUATION PROCESS The above listing of vacancies is supplementary to
the official posting of announcements, from which additional information

front and rear lobbies of the Radio Building, 325 Broadway in Boulder.
