I've played with the Infocentral family of php/apache/Mysql web apps:

Infocentral is moving off php, ChurchInfo is still under development,
and I haven't looked at OSC.  If all you need is simple people management
any of these should do well for you.

There is one flaw with ChurchInfo though.  Many of it's features are
baised toward families.  You can get around it for the simple features.
