It may be worthwhile to see if the kernel or file systems you use
have bug reports.

I've just had our email server lose access to a file system. 2nd time
in a couple of months. I suspect it is an xfs file system issue since
the kernel version is 2.6.17 which has had problems reported against
its xfs driver.
