We (PARSEC) do passive monitoring using NSCA as an interface to Nagios.
The NSCA daemon listens on TCP:5667 on the Nagios server and nsca_send submits submits check results from the monitored host.
In the service template for passively monitored hosts and services we have defined a staleness threshold to put the service into an UNKNOWN after 900 seconds of no report.
Every nsca_send must match a defined host:service pair.
The password encryption is pretty lame, but it might still be better than having no password at all.
There's a loose collection of facts that might answer your question or lead to further confusion or better questions. Let me know if any of those things happened, or if something else happened.
