MediaWiki seems to meet all your requirements plus it has an active
collection of extensions that can be added to your install:

MediaWiki is fairly strong on both. Two things of interest with
respect to authoring content via MediaWiki:
1. The Visual Editor project:
https://www.mediawiki.org/wiki/VisualEditor (which is available on
https://en.wikipedia.org if you login and uncheck "Temporarily disable
the visual editor while it is in beta" under your "Preferences" in
"Editing") This project is not quite ready for prime-time and would
likely be quite a lot to try to setup, but it is getting closer and
better all the time.
2. If you're a git user there is an mw-to-git program in the git
`contrib` directory
(https://github.com/git/git/tree/master/contrib/mw-to-git) (this is
also packaged for many distros) that allows you to download your
Wiki's content as a set of *.mw files that you can edit locally and
preview on your Wiki.
